Medicare card, DVA card, Pension card;
Information about your Private Hospital Insurance, if you have one;
Copies of results, X-rays, MRIs, CT scans and any other relevant information related to your condition.

Yes, you can, however you should know that consultation costs will not be covered by Medicare or your Private Insurance

Your GP needs essentially to write down a referral letter stating the reasons for the appointment and also what are the main concerns regarding your treatment.

Nowadays it is common to see patients looking for a second opinion. You should book an appointment keeping in mind that consultation fees for self-referrals are not claimable in part from Medicare or Private Insurance. In case of complex cases, please send the images (CT/MRI/PET) previous to the appointment. In many cases the efforts to find out better solutions comes from a detailed and thorough review of your images – which takes time and effort. Note: The time of the consultation includes my prior assessment of your imaging before seeing you and dictation of a letter to the referring doctor after your visit.
